25 | On 06/13/24 at 9:20 am, Licensing Program Analyst (LPA) Gerald Poindexter conducted an unannounced site inspection for the purpose of follow-up on an incident that occurred on 06/04/24 at around 11:40 am. The facility’s emergency backpack containing children’s records was left behind in the facility’s playground. The incident was self-reported to Community Care Licensing by the facility with a written incident report that was received within the seven day requirement on 06/07/24.
LPA Poindexter interviewed Director Kayla Thomas, Assistant Director Lauren Patlan, and reviewed related documents. The following ratios were observed today: 30 children from 2 pre-school and pre-k classrooms on the playground and inside the facility, supervised by 5 staff members; and 18 infants inside, supervised by 6 staff.
Based on information obtained, it appears that the staff (S1), a teacher, was on a playground area with an aide, supervising 15 Pre-K children. The emergency backpack, per facility policy, was brought outside to the playground. It was placed on a painting easel and left behind when the children and staff returned to the main building. The backpack, most notably, contained a first aid kit and children’s files for the 26 total students in that class. According to the director, all parents of children in that classroom have been notified of the incident. Director Thomas states that S1 was “written up” for the incident, which is being further reviewed by management.

Exit interview was conducted with Kayla Thomas, director. LPA reviewed and provided a copy of the report today. Notice of site visit and appeal rights were also provided. Notice of site visit shall remain posted for 30 days.
