13 | On 5/20/22 at 8:30 AM., Licensing Program Analyst (LPA), Adrian Mangina made an unannounced visit to deliver findings on the above-referenced allegations. LPA met with Director Michaela Serbin.
During this investigation, interviews were conducted, observation were made, and relevant documentation was reviewed. Based on the information obtained, LPA was unable to verify that staff are not properly changing day care child, as the facility has clear diaper changing guidelines and keeps a diaper log that is documented daily on a white board for parents to view. In addition, LPA was unable to corroborate the allegation that child incurred unreported injury while in care, since multiple parties transport the child to and from daycare and it is impossible to determine when or where the alleged injury occurred. Therefore, the above-referenced allegations are Unsubstantiated. A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. No deficiencies are cited. Notice of Site Visit was given and will remain posted for 30 days.
