25 | On 11/13/23 at 8:30 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced proof of corrections (POC) visit to the facility to follow up on a Type A deficiency cited on 11/6/23. Upon arrival, LPA was greeted by Assistant Director Shaun Bergmark and toured the facility. Census was as follows:
· Room #1 had 4 children with staff member Vanessa Torres
· Room #2 had 6 children with staff members Brynna Merezko and Alex Arballo
· Room #3 had 6 children with staff member Angelica (Norma) Delgado
· Room #4 was not in use
LPA observed appropriate ratio and capacity. LPA also observed appropriate care and supervision of children while they were engaged in morning activities. LPA inspected the brick ledges in classooms 1, 2, 3 and 4 to ensure they are covered appropriately and not accessible to children in care.
- In classroom #1, LPA observed 5 brick pillars. Three corner pillars are either inaccessible by furniture/cots or covered with adhesive foam. The two middle pillars have furniture around them, but are not completely inaccessible to children. LPA advises to apply adhesive foam to these two middle pillars.
- In classroom #2, LPA observed 2 brick pillars (one in the corner and one in middle of room). LPA observed the middle pillar has foam adhesive covering the bottom and sides of brick pillar. The corner is blocked by a basket of items and play kitchen. Both are inaccessible to children. (continued on LIC809-C...)
