On 1/12/23 at 11:15 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management visit for the purpose of providing a corrected report from a visit that happened on 1/9/23. LPA Lane recorded the report in error under the wrong facility number. Upon arrival, LPA met with Liz Guadarrama and toured the facility. Census was 21 children in 3 classrooms with 7 staff members. At this visit, LPA provided the corrected report to Director Liz Guadarrama, obtained signature and obtained incorrect report. LPA also obtained additional documents requested for the new director packet.
No deficiencies were cited at this visit.
Exit interview conducted and report was reviewed with facility representative Director Liz Guadarrama. Notice of site visit was given and must remain posted for 30 days.
