On 1/9/23 at 2:45 PM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management inspection for the purpose of following up on an unusual incident reported on 12/12/22. Director reported a child was accidentally enrolled in the toddler program before the child was age eligible. Upon arrival, LPA met with Director Liz Guadarrama and toured the facility. Census was 22 infants with 8 staff members in 3 classrooms.

LPA spoke with Director at facility regarding the unusual incident. Director stated that the child was enrolled in the toddler classroom by mistake as the date was written: day, month, year instead of month, day, year due to the enrollment coordinator working from Canada. The documents used by the facility uses the day, month, year format. On 12/12/22, Director requested an exception for the child to stay in the toddler classroom. However, center was closed due to the holidays from 12/16/22 through 1/3/23. Child was not immediately removed from the toddler classroom and as of today's date is now age-eligible for the toddler classroom. LPA advised in the future to double check birth dates to make sure they match all documents before placing child in classroom.

Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiency was cited (refer to LIC 809-D).

Exit interview conducted and report was reviewed with facility representative, Liz Guadarrama. A notice of site visit was given and must remain posted for 30 days.

Director stated she will provide a written statement verifying birth dates of children and also have the assistant director double check the birth dates as well to prevent this from happening in the future. Written statement was received during inspection.

Based upon Director statement, facility did not receive an approved exception from the Department before enrolling an infant in the toddler component which is a potential health, safety and personal rights risk to children in care.
