25 | On 11/20/24 at 9:00 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management inspection to follow up on an unusual incident that was reported on 8/15/24. Upon arrival, LPA met with Director Dolores Guadarrama and toured the facility. LPA observed a total of 32 infants/toddlers in the following classrooms:
· Room 6 had 9 infants (0-12 months) with three staff members
· Room 7 had 7 infants (12-24 months) with three staff members
· Room 5 had 11 toddlers (18-36 months) with two staff members
· Room 8 had 5 toddlers (18-26 months) with 1 staff member
Appropriate ratios and capacity were observed. LPA observed appropriate care and visual supervision during the inspection. According to the unusual incident report received and interview with Director, a toddler (C1) was left behind on the playground during a transition and was unattended for approximately 1 minute. According to the Director, there were other staff on the other playgrounds, but no one was immediately aware that C1 had been left on the playground. During the transition in the hallway, back to the classroom, it was then noticed that C1 had been left behind and a staff member went to retrieve C1 from the playground. It was determined that C1 was not injured, and the incident was reported to the parents and Licensing. Director provided further training and disciplinary action to the teachers responsible.
Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiency was cited (please refer to LIC809-D). Exit interview conducted and report was reviewed with facility representative Dolores Guardarrama. Notice of site visit was provided and must remain posted for 30 days.
