On 7/20/22, Licensing Program Analyst (LPA), Tyra Block, made an unannounced complaint visit for the complaint received on 5/10/22 for the purpose of delivering findings on the above referenced allegations. It was alleged staff hit a child on the back and left a child unattended on a changing table. Based on the information obtained during interviews and observations it is determined that the allegation is SUBSTANTIATED. The allegation is valid because the preponderance of the evidence has been met. The Type B deficiency is being cited on the attached LIC 9099D, cited in accordance with Title 22 California Code of Regulations.

An exit interview was conducted with Licensee, Jeanette Bradley. The Notice of Site Visit was provided and LPA advised Licensee it must remain posted for 30 days.

Licensee stated she will submit a written plan of action to discuss with staff Personal Rights and Parent's Rights with staff acknowlegment, in addition to terminating responsible staff (Staff #1). Plan of action will be submitted by POC due date of 7/21/22.
