On 12/29/22, Licensing Program Analyst (LPA), Tyra Block, conducted an unannounced Case Management to deliver 2 amended reports. LPA met with Jeanette Bradley who signed the amended reports. Present in Room 8 with facility representative were 13 children and a staff member.

This report along with a Notice of Site Visit (NOS) was provided, the NOS must be posted for 30 days.

An exit interview was conducted with facility representative, Jeanette Bradley.

All reports emailed as printer ran out of ink.
