On 8/24/2022 @ 9:45AM, LPA Nancy Diaz conducted an unannounced inspection and met with site director/licensee, Diane Martin. This inspection is in reference to facility's self-reported unusual incident involving a child who was diagnosed as having a nursemaid's elbow. On 7/13/2022, staff was holding a child by the hand when child pulled away from staff. Child initially stated that her arm hurt. Child was given an ice pack. Child appeared to be well the rest of the day. Child played, napped and ate normally. Child was not observed favoring arm.

A tour of the faciilty was conducted. Observed present today were 68 children in the following classrooms:

Foxes with 8 children and staff Leslie Yang & Ruby Zamarripa
Racoons with 6 children and staff Rakiesia Snyder
Kodiaks with 10 children and staff Carrie Miller & Mika Byfield
Grizzlies with 8 children and staff Emmalee Cobb
Fish with 10 children and staff Jenny Solachie
Bees with 9 children and staff Natalya Prehatny
Duck with 7 children and staff Lara Boilard
Butterflies with 10 children and staff Monica St. Claire

Staff who was involved in the incident was not present today. LPA obtained a signed staff declaration.

NO DEFICIENCY OBSERVED TODAY.
Exit interview was conducted with Diane Martine. A copy of this report and appeal rights were provided to Ms. Martine. Notice of site visit was observed posted. This shall remain posted for 30 days.
