On 5/12/23 at 2:20 PM, Licensing Program Analysts (LPAs) Keturah Lane and Gerald Poindexter conducted an unannounced case management inspection at request of Licensee. Licensee requested inspection to approve sink installation in preschool classroom #3 (Sprouts). Upon arrival, LPAs met with Director Sayra Godinez and toured the facility. Census was as follows:
Classroom #1 Buds had 9 children with staff members Ms. Sabrina Gutierrez and Ms. Ingrid Virgen Tamato
Classroom #2 Blossoms had 4 children with "Ms. Nayeli" (Joceline Herrera)
Classroom #3 Sprouts had 7 children with "Ms. Betty" (Graciela Morenosanchez)
Classroom Toddler #1 Seeds had 5 children with "Ms. Aleyda" (Annette Alvarez Acuna)

During inspection, LPAs observed proper ratios, capacity and supervision. LPAs inspected sink installation in classroom #3. Sink measured only 2 square feet in the classroom and did not affect the overall capacity number of 43. Capacity remains the same. During the visit, LPAs also observed cleaners in the cabinets in the upstairs children's bathroom that were accessible. Director stated the children are supervised when going to the bathroom and was unaware the items were in the cabinet. Items were removed immediately and placed in the locked cleaning supplies closet located upstairs. LPAs also observed two bolts sticking out of the fence (approximately 3-4 inches) on side area of playground that is not currently used by the children. Director stated she would remove them. LPAs have approved sink in classroom #3 for use.
No deficiencies were cited at this visit. Exit interview was conducted and report was reviewed with facility representative Director Sayra Godinez. Notice of Site Visit was provided and must remain posted for 30 days.
