On 10/06/2022 at 10:16am, Licensing Program Analysts (LPA), Samantha Clenista and Leilani Curtis conducted an announced Change of Ownership Pre-Licensing inspection. Upon arrival, LPA’s met with Co-Director, Alexis Segre. LPA's took a tour of the facility and observed a total of 34 children with a total of 5 staff members. This facility houses an infant, toddler and preschool program. After talking to Ms. Segre about the room names, the facility has requested to change the names to match updated sketch. The Toddler Program (children ages 18-36 months) operate in the Toddler 1 and Toddler 2 classroom. The Preschool Program (children ages 2-K) operate in the "Preschool Room" and "Library." Fire clearance was granted on 08/23/2022. There is a First Aid Kit kept in all classrooms. Medications are on site and are stored up high in a kitchen cabinet, inaccessible to children.

All indoor and outdoor activity space utilized for the children were inspected and measured today. The indoor play areas used for the infants, toddlers and preschool children were observed to be physically separate from one another. The following measurements were taken:
· Toddler 1- 118.92 sq. ft.
· Toddler 2- 452.88 sq. ft.
· Preschool Room- 1,250.06 sq. ft.
· Library- 55.88 sq. ft.

The total indoor activity space measured at approximately 1,876.74 sq. ft., which will accommodate a total of 53 children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. There is a 4 toilets, 2 urinals and 5 sinks available for the children. Children are to be escorted and supervised by staff to and from the bathrooms and under direct visual observation. All storage containers or trash cans containing solid or diaper waste have tight fitting lids and are in good repair.

There is a separate bathroom used by staff and is also designated for use by children who are ill. The director's office is the isolation area for children who are ill. There is not an operational carbon monoxide detector on site to be located. The kitchen area was observed free of rodents and/or vermin. All required documents for the parents and employees are posted. Sign-in sheet is located outside in front of the building. All children that were present during inspection were signed in. Ms. Segre stated there are no firearms/weapons on site. LPA’s observed no bodies of water on site.

The outdoor playground area was inspected and measured. Area was measured at approximately 2,830.06 sq. ft., which will accommodate no more than 37 children at one time. When LPA’s inspected the outdoor playground, LPA’s observed canopies and trees used for shade. Play area has age appropriate play equipment and sufficient cushioning under the climbing structure and is fully fenced (above 4ft. high). Drinking water is readily accessible inside and outside the classroom via water pitcher and refillable water bottles. Ms. Segre is reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

Incidental Medical services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and applicant discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov.

Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed the updated LIC311A with the facility representative today and reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. To receive important licensed-related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.

Prior to licensure, the following is needed:
  • Obtain and install an operational carbon monoxide detector
  • Submit a playground waiver request
  • Post all required postings near the entryway where parents/guardians can see
  • Obtain an updated fire clearance reflecting room name changes and capacity

Exit interview was conducted with Ms. Segre. LPA’s provided and reviewed all Provider Information Notice's regarding Lead Testing to Ms. Segre. Ms. Segre stated her understanding of the information and what is needed to be done by January 1, 2023. A license for 53 children (with 16 toddlers ages 18-36 months included) may be granted upon receiving all the above corrections and after a final file review. Ms. Segre stated her understanding. This report shall be kept available at the facility for public review for 3 years. Due to printer malfunction, LPA will email Ms. Hall a copy of this report and appeal rights at conclusion of visit.
