On 4/24/25 at 10:15 AM, Licensing Program Analysts (LPAs) Keturah Lane and Nancy Diaz conducted an unannounced complaint visit for the complaint received on 1/17/25, for the purpose of delivering findings on the above referenced allegations. Upon arrival, LPAs were greeted by staff member Stephanie Swanson and toured the facility. Director Coralito Garcia arrived at approximately 10:30 AM. LPAs observed a total of 7 infants in the infant classroom with staff members Evertina Salgado Arzate "Betty Garcia" and Stephanie Swanson.

It was alleged that staff are caring for infants in a space that hasn’t been cleared by CCL. Based on information obtained from text messages, photos, and investigative interviews with parents, children, and staff, 11 out of 19 individuals reported that multiple children were regularly cared for in the office area—some indicating this occurred two to three times per week—in order to avoid being out of ratio in the classrooms. The LPA also received a photo showing two daycare children in a play yard located in the office, which is not a licensed area for childcare. (continued on LIC9099-C...)

It was also alleged that a staff member handled an infant in a physically inappropriate way and used inappropriate language in the presence of a daycare child. According to information gathered from investigative interviews with parents, children, and staff, four individuals witnessed staff member (S1) aggressively placing an infant into a play yard, shouting an inappropriate word in Spanish at the director, and then storming off on January 8, 2025.

The allegations are valid because the preponderance of evidence has been met, therefore, the above allegations are found to be SUBSTANTIATED. See LIC9099D for deficiencies cited.

LPA Keturah Lane informed facility representative Coralito Garcia that this report dated 4/24/25 documents 1 Type A citation, which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Keturah Lane informed the facility representative to provide a copy of this licensing report dated 4/24/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with Director Coralito Garcia. Notice of site visit was provided and must remain posted for 30 days.

On 4/24/25 at 10:15 AM, Licensing Program Analysts (LPAs) Keturah Lane and Nancy Diaz conducted an unannounced complaint visit for the complaint received on 1/17/25, for the purpose of delivering findings on the above referenced allegations. Upon arrival, LPAs were greeted by staff member Stephanie Swanson and toured the facility. Director Coralito Garcia arrived at approximately 10:30 AM. LPAs observed a total of 7 infants in the infant classroom with staff members Evertina Salgado Arzate "Betty Garcia" and Stephanie Swanson.

It was alleged that staff does not ensure commercial disinfecting solutions are being used in accordance with label directions and that the changing table is cleaned after each use. Based on information gathered during investigative interviews with parents, children, and staff, interviewees reported that the cleaning solutions had a strong odor, but they could not confirm whether the products were used inconsistently with the label instructions. They also stated that diaper changing tables were cleaned after each use; however, the paper rolls used to cover the tables frequently ran out, and the owners often delayed restocking these supplies.
