On February 21, 2024, at 11:00 AM, Licensing Program Manager, Tashima Daniel and Licensing Program Analyst, Patrick Ma met with Center Director, Coralito Garcia and Licensees Satish Chandra and Shalini Raj Singh at the San Diego North Child Care Regional Office. The purpose of the meeting was to discuss the facility's Type A citation dated October 27, 2023.

On 10/26/2023, a complaint was received alleging staff roughly handled an infant. Complaint was substantiated on 10/27/2023 and was issued a Type A citation under Personal Rights, Section 101223(a)(3) due to a staff member yanking an infant’s arm twice and pressed/pushed roughly down on the back of the child’s head in an effort to force the infant to lay down for nap. The facility staff also failed to report this incident to licensing as required resulting in a Type B citation for Reporting Requirements, Section 101212(d)(1)(D).

It should be noted that the facility representative completed the Plan of Correction for Type A deficiency by the initial 10-day investigation visit on October 27, 2023 by terminating employment of staff identified violating the infant’s personal rights. Since the investigation, Director has also submitted a training agenda and sign-in sheet regarding additional training provided to staff on personal rights and supervision requirements.

Since licensure on November 3, 2022 the facility has been cited 3 times: 2 Type B’s and 1 Type A. All deficiencies have been cleared as of meeting date.

On August 30, 2023, Technical Support Program (TSP) was requested by then Director, Alexis Segri, and request was submitted by the Department on the same day. Ms. Garcia stated in the meeting that she met and received assistance with TSP Analyst over video conference/phone call in December 2023 and again in February 2024 with TSP Analyst Christopher Jackson, completing the services. RO and Director will receive TSP final report by the end of the week.

The following Regulations were reviewed and copies of each listed were provided to Licensee: 101223(a)(3) Personal Rights, 101212(d)(1)(D) Reporting Requirements, and 101217(a) Personnel Requirements. Child Care Advocate flyer was also provided during this meeting.

Facility representatives states they understand that they need to abide by Health and Safety Code and Title 22 Regulations in the operation of their child care facilities. Facility representative were advised to submit all Unusual Incident reports to incident reporting email: SDIncidentReports@dss.ca.gov.

Facility representatives were provided with CDSS Child Care Licensing (CCL) Family Child Care Providers Resource link with instructional videos: https://ccld.childcarevideos.org/family-child-care-providers/. It is recommended for facility representatives and staff to review the videos including, but not limited to: Supervising Children, Children's Personal Rights in Childcare, and Reporting Requirements. It is also recommended that facility provides quarterly in-service trainings to staff to keep them abreast of changing regulations and ensure familiarity of longstanding laws and regulations.

Licensee was advised to regularly visit the Community Care Licensing WEB SITE: www.ccld.ca.gov for quarterly updates and regulation. Facility representative stated they were all signed up for PIN’s updates and advised they could register additional emails here: https://www.cdss.ca.gov/inforesources/community-care-licensing/policy/provider-information-notices/child-care.
