13 | On 3/13/23 at 10:15 AM Licensing Program Analyst (LPA) Adrian Mangina made an unannounced complaint visit for the complaint received on 11/17/22 for the purpose of delivering findings on the above referenced allegations.
It was alleged that a child was injured by another child while in care due to lack of supervision and that the faclity failed to report the incident. Investigator conducted interviews with relevant parties and professionnals but based on the information obtained during the interviews and documentation reviewed it was impossible to determine when and where the injury occurred. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, therefore the above allegations are found to be Unsubstantiated. Exit interview conducted and report was reviewed with the facility representative, Victoria Petersen A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
