On 12/5/22 at 10:30am Licensing Program Analyst (LPA) Patrick Ma and Licensing Program Manager (LPM) Monica Cuddy made an unannounced visit for a Plan of Correction visit, to ensure compliance with the Cease-and-Desist Order provided on 10/12/22 and reiterated during an Non-Compliance Conference held on 11/16/22 with the Director Nicolle Daniels. During this meeting, they were provided 14-days to cease operation of the school-age program to allow parents time to make other child-care arrangements. Upon arrival, LPM and LPA were greeted by Nicolle Daniels. There was a large group of children ages 4-8 years old in the center lobby but moved to room 1A with Licensee Pam Thomsen when LPA and LPM arrived. After all the children moved into room 1A, there were 28 children in the room with Licensee Ms. Thomsen and Shannon Bignell.

There were no children upstairs where an unlicensed school age program was observed, cited and fined during previous visits. However, there were school aged children still in the facility co-mingled with the preschool children, violating the Cease-and-Desist Order and Health and Safety Order Code 1596.80. During site visit, Director applied for a Public School Affidavit. Licensee was advised that per previously issued cease and desist order, they are not to provide care to school aged children until the Private School Affidavit is approved by the California Department of Education. Licensee was cited today for continuing to provide unlicensed school age child care. A Letter for the Denial of the Application for School Age license was provided today.

See LIC 809D for deficiency cited. Licensee was also advised that once the Private School Affidavit is approved, they are not to commingle private school children with licensed preschool and infant program either indoors and outdoors. Director understand that she must be the full time director for the preschool and infant programs. Private school activities shall not interfere with Director duties and responsibilities.

Exit interview was conducted and report was reviewed with the licensee facility representative Nicolle Daniels.

Based on observation, facility continues to provided unlicensed care to school age children at the facility. Licensee stated there were chidren ages 4-8 yrs present during the visit. This poses an immediate health, safety and/or personal rights risk to children.
