On 03/10/23 at 10:15am, Licensing Program Analyst (LPA), Samantha Clenista conducted an announced Pre-Licensing inspection. Upon arrival, LPA met with CEO/Director, Marcie Laws. All indoor and outdoor activity space utilized for the children were inspected today. Applicant stated that the areas utilized for the school-aged children (ages 5-13yo) include the “Main Afterschool Classroom located in Building A. The fire clearance that was granted on 01/17/23. This facility houses an Adult Day Program from Monday thru Friday 8:15am-2pm, in Building B. The school-aged program and adult day program are in separate detached buildings, approximately 20 ft. walking distance from each other. Both indoor and outdoor activity spaces for each program are separate. LPA informed Applicant that the programs shall never commingle at any given time and that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor play. If medication is on site, Applicant stated that they will be in a locked box in the director’s office.

LPA continued to tour the facility and measured all indoor and outdoor activity space. The following measurements were taken:
· Main Afterschool Classroom measured at 2,014.68 sq. ft.
· Outdoor Space 1 measured at 1,155 sq. ft.
· Outdoor Space 2 measured at 22,174.8 sq. ft.

The total indoor activity space measured at approximately 2,014.68 sq. ft., which will accommodate a total of 57 children. LPA observed the indoor play space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. There is a total of 3 toilets, 1 urinal and 4 sinks available for the children. There is a separate bathroom used by staff that consists of 1 toilet and 1 sink and is also designated for use by children who are ill. The director's office is designated to be the isolation area for children who are ill. There is an operational carbon monoxide detector on site located in the kitchen area/food prep area and in the main room.

The kitchen area currently includes one refrigerator, one sink, storage cabinets, and one toaster oven. Kitchen area and food storage area was observed free of rodents and/or vermin. All required documents were observed posted in the main parent drop off/pick up area. This area is also where the electronic (via ProCare) sign-in and out is located. The two outdoor play areas were measured and inspected.

The total outdoor play space measured at approximately 23,024.81 sq. ft., sufficient for 306 children. Both outdoor spaces are physically separate from the space utilized by the adult day program. Outdoor Space 1 had a gazebo and lunch tables under it. This area was observed to be fully fenced in via 5ft. metal fencing. Outdoor Space 2 had a big basketball court, grassy areas, and gazebo with a table under it. There is a animal pen that had 3 goats in it and was observed to be fenced off. Applicant stated that she has play equipment that will be brought outside during outdoor play. LPA observed there to be sufficient play equipment that was age appropriate. Outdoor Space 2 had the gazebo and trees used for shade. Outdoor Space 2 was observed to not be fully fenced via 4ft. fencing. There is a small area in front of Building A and B where the fencing is shy of 4ft. Drinking water is readily accessible inside and outside the classroom via refillable water jugs and water cups. Staff #1 is pending Livescan clearance and Applicant is informed that Staff #1 shall not work at the facility until her clearance is received.

Incidental Medical services (IMS) was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed the updated LIC311A with the facility representative today and reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children.

Prior to licensure, the following is needed:
  • Get a lead clearance since the Applicant tested the water faucets and the faucet in the kitchen exceeded the 5.5 ppb level. Applicant stated that they have been flushing it within the past 2.5 weeks and has an appointment next Tuesday (03/14/23) to have the faucet retested.
  • Remove or make hazardous items inaccessible that are located in the Outdoor Space 2 near the animal pen
  • Extend the fencing located between Building A & B to meet 4ft. or higher, to ensure Outdoor Space 2 is fully fenced via 4ft. fencing
  • Submit an update facility sketch showing updated outdoor play space


Exit interview was conducted with Applicant. A school-aged license for 57 children (ages 5-13) in the “Main Afterschool Classroom” may be granted upon receiving the above corrections and after a final file review. Applicant stated her understanding. This report shall be kept available at the facility for public review for 3 years.
