On 02/23/2023 at 9am, Licensing Program Analyst’s (LPA), Samantha Clenista and Adrian Mangina conducted an announced Pre-Licensing inspection. Upon arrival, LPAs met with CEO/Center Director, Alexandra Camacaris and her husband, Botta Kol. LPA’s took a tour of the facility per facility sketch. There were no children present during inspection. The Preschool Program (children ages 2-K) will operate in Classroom A and Classroom B. Fire clearance was granted on 01/19/2023. There is a First Aid Kit kept in the director’s office. When medications are on site, they will be stored in the director’s office (inaccessible to children).

All indoor and outdoor activity space utilized for the children were inspected and measured today. Classroom A measured at approximately 567.55 sq. ft. which will accommodate a total of 16 children. Classroom B measured at approximately 698.32 sq. ft. which will accommodate a total of 19 children. Total square footage for indoor play space is 1,265.87 sq. ft., sufficient for a total of 36 children. LPA’s observed all indoor play space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. There are a total of 4 toilets and 5 sinks available for the children. Children are to be escorted and supervised by staff to and from the bathroom and under direct visual observation. There is a separate bathroom used by staff located in the Main House which will also designated for use by children who are ill. A small area in Classroom A will be designated as the isolation area for children who are ill. There is a carbon monoxide detector on site located in both Classroom A & B (plugged into the ceiling). The kitchen area is located in the Main House, which currently includes a microwave, oven, refrigerator, and sink. Facility representatives stated they do not plan to open until the summer (June 2023). LPA’s advised that prior to enrolling children, adequate food and snacks shall be present. Sign-in sheet is will take place upon entry of the facility via Brightwheel using an iPad.

The facility has a total of 5 outdoor play areas per facility that were inspected and measured. Play Area 1 measured at 1,914 sq. ft., Play Area 2 measured at 1,122.3 sq. ft., Garden Area measured at 1,222.49 sq. ft., Lunch Area measured at 1,542 sq. ft., and Play Area 3 measured at 1,751 sq. ft. The total square footage for all outdoor play areas measured at approximately 7,551.79 sq. ft., which will accommodate no more than 100 children at one time. When LPA’s inspected the outdoor play areas, LPA’s observed canopies and trees used for shade. There is a shade used in Play Area 1 that was observed broken due to the storm, that shall be repaired prior to licensure. Play Area 2 has a play structure that does not have an age appropriate verification for it. LPA’s observed sand underneath the play structure, however, does provide the appropriate cushioning for a fall. All outdoor play areas are completely fenced in via wooden planks that are more than 4ft. high. Drinking water is readily accessible inside and outside the classroom via refillable water jugs and refillable water bottles.

Incidental Medical services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA’s reviewed the updated LIC311A with the facility representative today and reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Prior to licensure the following shall be corrected:
· Obtain age verification for the big play structure located in Play Area 2
· Place enough cushioning under the play structure located in Play Area 2
· Make the exposed stump that has mushrooms located in Play Area 3 inaccessible
· Make the uneven tile located outside in the lunch area safe as it causes a tripping hazard
· Have a thermometer as part of their first aid kit
· Remove the cabinet doors under the children sink in Classroom B
· Obtain an updated fire clearance reflecting 36 children instead of 32 children
· Repair the shade that is located in Play Area 1

Exit interview was conducted with Facility Representatives. LPA advised that Facility Representatives are required to have the water tested and results posted within 180 days of licensure. A preschool license for 36 children may be granted upon receiving the above corrections and final file review. Facility Representatives stated their understanding. This report shall be kept available at the facility for public review for 3 years.
