On 03/22/23 at 9:20am, Licensing Program Analysts (LPA), Samantha Clenista and Selina Siao conducted an announced Pre-Licensing inspection. Upon arrival, LPAs met with Director, Leslie Scheck. All indoor and outdoor activity space utilized for the children were inspected today. Director stated that the main indoor area utilized for the school-aged children (ages 5-6yo) is Room #10. The fire clearance was granted on 03/08/23. This facility houses a toddler and preschool program. LPAs informed Director that the programs shall never commingle at any given time and that direct visual supervision of the children shall be maintain at all times during indoor and outdoor play. Midway of the inspection, Director requested a change of room usage for the school-aged children from Room #10 (downstairs) to Room #23 (upstairs). Director is aware that an updated fire clearance will need to be requested and approved prior to licensure. Director provided LPAs with an updated facility sketch reflecting changes.

LPA measured Room 23 and it measured at approximately 539.9 sq. ft. sufficient for 15 children. Room was observed not set up yet and Director stated she will provide LPA pictures of it after it is set up. During LPAs inspection, Director stated that she would also like to add an upstairs room as an additional play space called the Multi-purpose Room (aka Beit Midrash room) and Sanctuary (used for chapel purposes). This area is to be shared with the toddler and preschool program, however not included in the overall square footage. An updated fire clearance and waiver to share these rooms shall be requested and approved prior to usage. Facility has two separate restrooms (boys and girls) that have a total of 5 toilets, 3 urinals and 4 sinks that afford privacy. There is a separate bathroom used by staff that consists of 1 toilet and 1 sink and is also designated for use by children who are ill. The office is designated to be the isolation area for children who are ill. There is an operational carbon monoxide detector on site located in Room #3 and in the office hallway. The kitchen area currently includes a refrigerator, sink, microwave, oven, toaster oven, dishwasher and storage cabinets. Kitchen area and food storage area was observed free of rodents and/or vermin. All required documents were observed posted in the main office. The sign-in and out is located in each classroom.

The outdoor play area (Yard 2) was measured and inspected. This area is a shared space between the school-aged children and preschool children. A waiver to share the playground at different times must be submitted and approved prior to licensure. The outdoor play space measured at approximately 1,775.5 sq. ft., sufficient for 23 children. This area was observed to be fully fenced in via 4ft. metal fencing. Area has age appropriate and safe play equipment that was present. Drinking water is readily accessible inside and outside the classroom via refillable water jugs and water bottles.

Incidental Medical services (IMS) was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and applicant discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov.

Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed the updated LIC311A with the facility representative today and reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children.

Prior to licensure, the following is needed:
  • Request for a playground waiver to share Yard 2 with the older preschool children at different times
  • Obtain an updated fire clearance to reflect that the facility will utilize Room 23 upstairs and to add the Multi-purpose Room and Sanctuary
  • Request a waiver to share the Multi-purpose Room and Sanctuary with the toddler and preschool program
  • Set up Room 23 and send pictures once it is set up
  • Submit a waiver due to the facility having an armed guard on site

Exit interview was conducted with Director. A school-aged license for 15 children (ages 5-6 years old) in Room #23 may be granted upon receiving updated fire clearance, the above corrections and after a final file review. Director stated her understanding. This report shall be kept available at the facility for public review for 3 years.
