25 | On 2/28/25 at 9:00 am Licensing Program Analyst (LPA) Gerald Poindexter conducted an unannounced case management inspection to follow up on an unusual incident reported by the facility that occurred on 1/7/25. According to the report submitted by the facility, a daycare child (C1) was found unattended in the hallway outside the child’s classroom, Butterflies (infant classroom 2, also directly next to the Director’s Office), following transition from the playground. LPA met with Lauren Patlan and toured the classrooms. LPA observed a total of 46 children (16 infant, 30 pre) with 13 staff (6 infant, 7 pre) in 5 classrooms.
During this inspection, LPA interviewed one staff member, S1. Based upon interview and the incident report provided by the facility, C1 was left unattended for approximately 2 minutes in a hallway, while strapped inside a stroller. Staff S1, recognized the child was missing and the child was returned to class. Parents of C1 were notified of the incident by the facility. Since C1 was found shortly after a headcount by the staff within the facility and within view of other staff, a Type B deficiency was cited for a potential health and safety risk.

Exit interview conducted and report was reviewed with the Lauren Patlan, center director. A Notice of Site visit was given and must remain posted for 30 days. Appeal Rights provided.
