On 09/22/23 at 9:45am, Licensing Program Analyst (LPA), Samantha Clenista conducted an announced Pre-Licensing inspection. Upon arrival, LPA met with CEO, Christine D'Amico. All indoor and outdoor activity space utilized for the children were inspected today. Facility is requesting to be licensed for 30 children (ages TK thru 12-years-old) in Room B8 located on Clay Elementary school site. Fire clearance was granted on 08/14/2023. When medications are on site, Ms. D'Amico stated that they will be in a locked box located in the classroom. Facility is exempt from indoor and outdoor square footage per Title 22, sections 101538.2(c) and 101538.3(c). No measuring of any sort was conducted today.

LPA inspected all indoor and outdoor play space. Age-appropriate toys, supplies and equipment were observed available in the indoor activity space. Facility is requesting to utilize a total of 3 separate outdoor play areas which include the large blacktop area, grassy play space (between Room 8 and 3) and Kindergarten Playground. Ms. D'Amico stated that the children in her program will not commingle at any given time with children from other programs (i.e. PrimeTime). There are trees and canopies used for shade. There is rubber cushioning under the play structures where there are falling hazards. LPA observed that two of the play structures located on the black top area are for ages 5-12 years old. Facility is requesting to be licensed for ages TK-12. Ms. D'Amico stated she will ensure that staff provide additional supervision and alternative play equipment for children who are less than 5-years-old and is in TK.

Facility will be utilizing two separate children restrooms (which allows individual privacy) located near the blacktop area, that consists of a total of 4 toilets, 3 urinals and 4 sinks. Toilet facilities shall not be utilized simultaneously by children of both sexes. Children are to be escorted to and from the restrooms at all times. There is a separate staff restroom located near the lunch arbor per facility sketch, labeled "Staff RR" which will also be designated as the isolation restroom.

Isolation area is located in a small area closest to the door. Facility requires all parents to bring their children snacks for the PM, however facility will maintain extra snacks in case someone forgets. Cleaning supplies and other toxics are inaccessible to children through latches, locks and/or placed on high surfaces. Due to the facility sharing Room B8 with the school district at separate hours, Ms. D'Amico is reminded to ensure a room check is conducted prior to having children present to ensure that anything that reads, “Keep Out of Reach of Children” is inaccessible to children. Drinking water is readily accessible via Brita filter and refillable water bottles.

LPA reviewed with Ms. D'Amico the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Prior to licensure, facility shall obtain sterile dressing, adhesive tape, and scissors to have a complete first aid kit and place battery in carbon monoxide detector to make it operational. Exit interview was conducted and report was reviewed with Ms. D'Amico. Notice of Site Visit was provided and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Ms. D'Amico was reminded of the requirement to have the water tested and results posted within 180 days of licensure. A school-aged license for 30 children (ages TK thru 12-years- old) in B8 may be granted upon receiving the above corrections and after a final file review. Ms. D'Amico understands that this report shall be kept available at the facility for public review for 3 years.
