13 | On 7/22/26 at 8:45 am, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced complaint inspection to deliver findings regarding a complaint received on 6/16/26. LPA met with Direcor Jiovanna Ruiz Quintero. During the investigation, LPA toured the facility, observed children in care, reviewed relevant documentation, & interviewed the facility director, staff, & parents.It was alleged that staff prevented the child from receiving health-related services at the facility. During the inspection, staff were observed appropriately supervising and interacting with the children. Interviews and records reviewed provided conflicting information and did not corroborate the allegations. LPA was unable to interview the child’s behavioral therapist, and no independent witness statements or documentation were obtained to confirm the alleged incidents. Although the allegations may have occurred or may be valid, based on the information obtained, there is not a preponderance of evidence to prove or disprove that the alleged violations occurred. Therefore, the allegation is UNSUBSTANTIATED. An exit interview was conducted, & the report was reviewed with the facility director. A Notice of Site Visit was provided & must remain posted for 30 consecutive days. |
