1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 02/12/24 at 9:50am, Licensing Program Analysts (LPA), Samantha Clenista and Patrick Ma, conducted an announced Change of Location Pre-Licensing inspection. Upon arrival, LPAs met with Facility Representatives Elena Alvarado (Center Director), Cecilia Baeza (Assistant Director) and Victoria Chavez (Education Manager). All indoor and outdoor activity space utilized for the children were inspected today. Facility is requesting to be licensed for 140 children (ages 2-5 years old) in the following rooms: 1, 2, 3, 4, 5, 6, 7 and 8. Fire clearance was granted on 01/25/24. LPAs informed facility representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor play to ensure the health and safety of all children. When medications are on site (depending on the medication), Facility Representatives stated that they will be in a locked cabinet within each classroom. LPAs continued to tour the facility and measured all indoor activity space. Total indoor activity space measured at approximately 3,387.32 sq. ft., sufficient for only 96 children. Facility is requesting for 140 children. There is a total of with 10 toilets and 13 sinks available for the children. LPAs observed all indoor play space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. There are two separate staff restrooms located in the building hallways (labeled as #1 and #2 on the sketch). Bathroom # 1 is equipped with 1 toilet, 1 sink and 1 urinal, which will also be the bathroom used for children who are ill. Bathroom #2 is equipped with 1 toilet and 1 sink. Facility Representatives stated that the Director''s office will be the isolation area for children who are ill. The kitchen area currently includes two refrigerators and 1 sink that has hot and cold water. Kitchen area and food storage area was observed free of rodents and/or vermin. There is an operational carbon monoxide detector on site located in each classroom. All required documents were observed posted in the main parent drop off/pick up area. This area is also where the electronic (via Child Plus) sign-in and out is located. Drinking water is readily accessible inside and outside the classroom via filtered water faucet within each classroom and refillable water jugs available for the children.

Facility representatives stated they will provide the children with refillable water cups. Facility Representatives were reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

The facility currently has one playground located in the back of the building, however when LPAs were inspecting the area, there were several concerns prior to LPAs measuring. LPAs observed the playground to have multi-leveled areas with stairs and slopes that lead to certain areas that the facility would like to utilize as play space. LPAs informed Facility Representatives that all hazardous areas must either be fenced off and/or gated to ensure the overall health the and safety of all children playing outside. LPAs also observed that the green fencing surrounding the playground is shy of 4ft. high. Facility Representatives stated that they will relay the information to upper management and will let LPAs know what they plan on doing as far as what areas they are willing to fence off. A follow-up inspection will need to be conducted after the fencing/gates are installed so that LPAs can obtain measurements.

LPAs also observed there to be numerous wooden logs, childlike picnic tables and playground structures that were observed to be smooth to the touch during time of inspection. Facility Representatives were advised to conduct ongoing inspections of all wooden structures and play equipment to ensure no areas present a safety hazard, including but not limited to possible splintering or tripping. Facility Representatives were also informed that children must be escorted to and from the playground, and that staff must maintain direct supervision at all times. Area had age-appropriate toys and play equipment and had wooden chips under play structures, sufficient to sustain a fall.

After touring the entire facility, LPAs observed a running water fountain located right of the childcare center building, in between the addresses of 511 and 489 Saxony Place. The fountain was observed to be running and filled with approximately 1.5 ft of standing water. To ensure the health and safety of all children, Facility Representatives shall ensure that this water fountain is inaccessible per Building and Grounds regulation 101238(e). LPAs reviewed the regulation with Facility Representatives and presented some options to them. Facility Representatives stated they will review everything, consult with their management and will notify LPAs once they figure out how they will correct it.

LPAs reviewed with facility representatives the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Facility representatives were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Facility representatives were informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Prior to licensure, the following is needed:

Ensure all outdoor play areas are appropriately fenced and safe for children.
Ensure the water fountain that is located right of the building between address 511 and 489 Saxony Place is inaccessible per Building and Grounds regulation 101238(e).
Submit updated facility sketch reflecting the following areas; observation room, isolation restroom/area, update number of children sinks and toilets.

Exit interview was conducted and report was reviewed with Facility Representatives. A notice of site visit was given to Director, Elena Alvarado, and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Facility Representatives were reminded of the requirement to have the water tested and results posted within 180 days of licensure. A preschool license for children (ages 2-5 years old) in Rooms #1-#8 may be granted upon receiving the above corrections and after conducting a final file review. Capacity of children served shall be decided after LPAs measure the outdoor play space once corrections are completed. Facility Representatives stated that they will have all corrections submitted to LPA Clenista no later than March 1, 2024. Facility Representatives understands that this report shall be kept available at the facility for public review for 3 years.
