On 03/28/2024 at 9:40am, Licensing Program Analyst (LPA), Samantha Clenista conducted an announced Pre-Licensing inspection for a Change of Ownership. Upon arrival, LPA met with Facility Representatives, Drake Oswald (Operations Director) and Tantiani Budiman (Center Director). Applicant is requesting to be licensed for 124 preschoolers (ages 2 to entry/attending of kindergarten including TK) in classrooms 1, 2, 3, 4, 5, 6, 7, 8 and 9. Hours of operation will be Monday thru Friday, 7am-6pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Facility Representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities to ensure their overall health and safety. When medications are on site, Facility Representatives stated that they are kept in a locked cabinet located in the office. A fully equipped first aid kit is located in the meeting room and in each classroom. There is an operational carbon monoxide detector on site located in the office. All required licensing documents were observed posted in the main office. Children will be signed in and out in the office area via electronic system called Brightwheel.

LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 5,806.59 sq. ft., which is sufficient to accommodate the requested overall capacity of 124 children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via refillable water jugs and water bottles. Facility had their water tested on October 17, 2023, and no lead exceedance was found. LPA obtained a copy of the results during time of inspection.

LPA observed all hazardous items to be inaccessible to children via locked cabinet or placed up on high surfaces. LPA reminded Facility Representatives and staff that were present to ensure daily checks of all classrooms and outdoor play areas are free of hazards, especially anything that reads “Keep out of Reach of Children.” There are no bodies of water or weapons on the property. Fire clearance was granted on 03/06/24.

LPA observed a total of 18 sinks and 17 toilets available for children’s use. These are sufficient to accommodate the requested overall capacity of 124 children. There are several staff restrooms located on site with a total of 8 toilets, 7 sinks and 3 urinals. The isolation area for children who are ill will be the office and one toilet/sink located in the woman’s restroom located between Classroom #3 and #4. Facility sketch shall be updated to reflect this change. Facility Representatives were reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

Facility will provide two snacks (AM and PM) and one lunch meal. The kitchen area currently includes a sink, 2 ovens, 2 refrigerators and 3 freezers. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials.

The facility currently has 3 separately fenced playground areas available for the children (Playground #1, #2 and #3 per facility sketch). Fencing of the entire campus is a mixture of chain link and vertical rot iron which was observed to be at least four feet high. The total square footage for Playgrounds #1-#3 is 21,793.06 sq. ft., which is sufficient to accommodate the requested capacity. Shade is provided via canopy sails and trees. There are sufficient outdoor age-appropriate toys and play equipment available on the playgrounds. There were several climbing structures observed on all playgrounds that were observed to be in good condition, properly anchored, however no age verification was on them. There were two main structures (Playground #1 and Playground #3) where LPA observed there to be insufficient cushioning under the fall zones. Facility stated they will follow up with a playground inspector and explore solutions of how to keep it up to code. Drinking water is available via refillable water jugs and water bottles. LPA observed various walkways that lead from the main office and Classroom #1 to the fenced playgrounds.

There are other classrooms and staircases accessible, however, Facility Representatives stated that children are escorted and directly supervised to and from the fenced playgrounds at all times. All hazardous items on the playgrounds were observed to be inaccessible to children. The facility would like to add their garden area as part of their outdoor activity space, however, LPA observed several hazards that are accessible. Facility Representatives stated they will fence it off so that it is fully enclosed when the children are gardening. LPA will conduct a follow-up inspection after the fencing has been installed to inspect and measure the additional outdoor space.

Facility Representatives were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with Facility Representatives the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Facility Representatives were informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given to Facility Representatives and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Exit interview conducted and report was reviewed with the Facility Representatives, Mr. Oswald and Ms. Bundiman.

The following corrections are needed prior to the issuance of the license:
· Fence off the garden outdoor area to ensure all hazards are inaccessible.
· Make the art supply room door inaccessible located in Classroom #3.
· Relabel the classrooms so that it matches the facility sketch.
· Update the sketch to reflect updated isolation toilet/sink.
· Post updated facility sketch in main lobby.
· Place labels on isolation toilet/sink.
· Place sufficient cushioning under climbing structures located in Playground #1 and #3.
Facility Representatives stated they understand that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. If an extension is required, Facility Representatives will contact LPA to request it and provide an explanation for the extension.
