13 | On 1/7/26 at 8:30 AM, Licensing Program Analyst (LPA) Mahjoba Mohsini made an unannouced complaint visit for the complaint received on 11/5/25 for the purpose of delivering findings on the above refrenced allegationt. Upon arrival, LPA met with designated facility representative (Patricia Juaregui) and Director (Kelly Lhungay). LPA observed 30 children and 5 staff as follows:
• 20 children at the play ground with staff (Daisy Lewis, Mia Melideo & Patricia Jauregui)
• 10 children in (Toddlers Classroom) with staff (Alison P. William, Neyla Brito)
Facility is within ratio and capacity.
It was alleged Staff is not allowing parent to access the child’s records. Based on the information obtained during the interviews, observations, and documents reviewed, it is determined that the facility director acted in accordance with the regulations. Although the allegation may have happened, there is not a preponderance of evidence to prove that the alleged violation occured, therefore, the above alllegation is found to be Unsubstantiated.
Exit interview conducted and report was reviewed with facility representative Kelly Lhungay. Notice of site visit was given and must remain posted for 30 days. Appeal rights were verbally discussed and provided. |
