On 8/6/25, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced visit for a complaint received on 7/8/25. The LPA met with Owner, Sandra Reyes. There were 32 children and 10 staff present today.

Based on the information obtained from file review, parent and staff interviews, it was determined that a facility staff member yelled at a day care child. The interviews revealed sufficient information to prove that a staff member yelled at a day care child. The preponderance of the evidence has been met and therefore, the above allegation is found to be SUBSTANTIATED.

Type B Violation was cited. Refer to the next page LIC 9099-D for deficiency. The exit interview was conducted with the Owner, Sandra Reyes. The Appeal Rights and a copy of the licensing report were provided. A notice of site visit was posted and must remain for 30 days.
