On 4/23/26, at 8:30am Licensing Program Analyst, Annette Sutherland, made an unannounced visit to deliver findings on the above referenced allegation. During the investigation, LPA conducted interviews with staff and parents and reviewed all relevant documentation. Based on the information obtained, it was determined that an unreported incident occurred in October 2025 and again in November 2025. Staff did not report the incidents to parents or to the director until mid December 2025. Staff have since been retrained and understand that they must report incidents to director and parents of child involved in a timely manner. The preponderance of evidence standard has been met; therefore, the above allegation is found to be Substantiated and a Type B citation under California Code of Regulations, (Title 22, Division 12 & Chapter 1) is being cited on the attached LIC 9099D. NOTICE OF SITE VISIT WAS GIVEN AND WILL REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director, Leila Ebtekari. A notice of site visit was given and must remain posted for 30 days

On 4/23/26, at 9:30 am Licensing Program Analyst, Annette Sutherland, made an unannounced visit to deliver findings on the above referenced allegation. During the investigation, LPA conducted interviews with staff and parents and reviewed all relevant documentation. Based on the information obtained, it was alleged that an incident occurred in October 2025. During the investigation, LPA conducted interviews with staff and parents and reviewed all relevant documentation. The facility reported that sunscreen is not provided on site unless a child has specific written authorization and brings sunscreen from home for individual use. Based on the information gathered, the LPA was unable to obtain corroborating evidence to support or disprove the allegation. Although the allegation may have occurred or may be valid, there is not a preponderance of evidence to substantiate that a violation occurred. Therefore, the allegation is determined to be Unsubstantiated. An exit interview was conducted, and the report was reviewed with the director Leila Ebtekari. A Notice of Site Visit was provided and must remain posted for 30 days.
