13 | On 4/23/26, at 9:30 am Licensing Program Analyst, Annette Sutherland, made an unannounced visit to deliver findings on the above referenced allegation. During the investigation, LPA conducted interviews with staff and parents and reviewed all relevant documentation. Based on the information obtained, it was alleged that an incident occurred in October 2025. The facility reported that sunscreen is not provided on site unless a child has specific written authorization and brings sunscreen from home for individual use. Based on the information gathered, the LPA was unable to obtain corroborating evidence to support or disprove the allegation. Although the allegation may have occurred or may be valid, there is not a preponderance of evidence to substantiate that a violation occurred. Therefore, the allegation is determined to be Unsubstantiated. An exit interview was conducted, and the report was reviewed with the director Leila Ebtekari. A Notice of Site Visit was provided and must remain posted for 30 days. |
