 On the above date and time, Licensing Program Analyst (LPA) Kelly Gerth arrived unannounced at the facility and met with Site Supervisor Leticia Gonzalez, to conduct a plan of correction visit for deficiency: 101238.3(a), Issued on: January 10, 2025 with a plan of correction due date extension of January 29, 2025.

As of the date and time of this visit, the Plan of Correction for deficiency 101238.3(a) has been met as evidenced by:

During the tour of the facility with Site Supervisor Leticia Gonzalez, LPA Gerth observed the fire/alarm system and all door handles and locks, to be in working order.

An exit interview was conducted with Site Supervisor Leticia Gonzalez, and appeal Rights were discussed and provided along with a copy of this report.

A Notice of Site visit was given, Site Supervisor Leticia Gonzalez, was reminded the notice must remain posted for 30 days.
