(1) A required – 1 year inspection is being conducted as part of a compliance review. Licensing Program Analysts (LPAs), Susan Brewer, James Wilkerson and Jeanette Sanchez, arrived at the facility for the purpose of conducting an Annual Inspection. LPAs were greeted by Director Jesusa Polk, and granted entry to tour center, inside and out. The following was observed: A census was taken of 2 staff and 9 preschool children present.
· A review of the staff records and review of a sampling of children's records were conducted as part of this evaluation. See Confidential Names List (LIC811)
· The licensee is asked to update the following documents, if applicable, and submit to licensing within 30 days:
1. LIC 500 Personnel Report
2. LIC 610 Emergency & Disaster Plan
3. LIC 309 Administrative Organization (only if changes have been made)
4. LIC 308 Designation of Administrative Responsibility (only if changes have been made)

· Classrooms are clean and free of hazards
· No weapons stored at the facility
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· No Medications are stored at the facility on 07/20/2021
· Hazards are stored where inaccessible to children which include: Disinfectants, cleaning solutions and other items that are dangerous on 07/20/2021
· Poisons and toxins are locked
· All floors shall be clean and safe
· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are enclosed by appropriate fences and free of hazards
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair
· Menus shall be posted at least one week in advance in a place visible by the child’s authorize representative, dated and kept on file for 30 days, and made available upon request
· Uncontaminated drinking water shall be readily available both indoors and out and provided by water dispenser, disposable cups and water pitchers.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall, with wood chips.
· At approximately 8:46 AM, Sign in/Sign out record was reviewed and does not meet regulation requirements due to authorized adults only using initials to sign 4 children in and out of the facility.
· A Staff member is present with current Pediatric CPR/First Aid which expires on 03/2023
· Opening and closing staff member’s CPR/First Aid expires on 04/2022 and 08/2022
· Director completed Health and Safety Training on Summer of 2001.
· The Licensee was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· Required records for children shall ensure that each child’s record contain a medical assessment and contain the Identification and Emergency Information

· Required records for staff shall ensure that each personnel record contain a health screening, verified.
· Documentation of fire & earthquake drills to be conducted every six months. Last fire drill on 07/19/2021 at 10:30 AM, with 24 children present and 7 staff.
· A review of staff records on 06/23/2021 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov
· Facility is not currently providing IMS Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided an updated Plan of Operation that includes IMS must be submitted to the Department. The following was also reviewed and discussed:
v AB 1207 – Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018In accordance with California Health and Safety Code Section 1596.8662 – requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. Existing licensees must meet requirements by March 30, 2018. New employees shall have 90 days to complete training as required. This training requirement may be directly met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com/ and are provided in English and Spanish. If no training is made available in a required person’s primary language then those persons shall be exempt from this requirement

v Effective January 1, 2017 – Children under 2 years of age shall ride in a rear-facing car seat unless the child weighs 40 or more pounds OR is 40 or more inches tall. For additional information regarding car seat laws see www.chp.ca.gov

v AB2960 – Child care and development services, online portal – Effective June 20, 2022 - This bill requires the State Superintendent of Public Instruction (SSPI), within the California Department of Education (CDE), to develop and post on CDE's website a comprehensive child care and development services online portal for families and providers by June 30, 2022. Further, the bill would require the SSPI to submit a report to the Legislature by January 1, 2021, proposing plans for enhancements to the online portal and inclusive of a comprehensive implementation plan, based on recommendations from the workgroup.

No Civil Penalty has been assessed during this inspection.
No deficiencies cited.

An exit interview was conducted and during the interview, the licensee, Susan Jesusa, confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.

A NOTICE OF SITE VISIT WAS ISSUED AND LPA VERIFIED THAT IT WAS POSTED IN A PROMINENT LOCATION AT THE FACILITY BEFORE LEAVING. THE LICENSEE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS.

This report must be available for review, upon request, for the next 3 years.
