On May 25, 2022 at 10:55 am Licensing Program Analysts (LPAs), Ana Noble and Linda Almaraz conducted an Case Management visit regarding room changes that have taken place due to requested changes in capacity to other programs located at this site (School Age #376300052). The capacity will be decreased from 42 to 35.
The designated Preschool Classroom are #3, 4 and 5 for a total capacity of 35 Preschool Children. Capacity change will be processed once Increase in capacity for school age is granted.

Waiver for shared playground with School Age Program, granted and on file.

No deficiencies cited.

An exit interview was conducted, appeal rights and notice of site visit was issued to Director, Susan Polk.

THE LICENSEE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS.
