32 | While some children may have been isolated only when staffing permitted, this protocol was not followed consistently. Interviews corroborated that emergency contacts are not called, children were not picked up and were routinely allowed to stay until end of day. Record review verified inconsistencies with child absence due to illness and returning to CCC prior to a 24-hour return policy or without a doctor note, subjecting other children and staff to potential exposure.
The third allegation alleged that staff do not keep the facility free of insects. It was disclosed that on multiple occasions, cockroaches have been found in classroom cabinets, closets, storage areas and observed to be walking across classroom carpets. Record review verified that management has contacted CCC maintenance team to provide temporary traps, however no documentation was provided to corroborate that follow-up was requested from district approval to authorize at minimum a yearly exterminator service. With insects being found within areas where children play, crawl, sleep and eat, routine services are required as a proactive and preventative measure, to decrease the probability of insect infestation.
Based on interviews, observations and record review, the preponderance of evidence standard was met, and the allegations are substantiated. Violations of California Code of Regulations, Title 22, sections 101226.2(b), 101226(a)(1) and 101238(a)(1) will be cited on the attached LIC 9099D.
An exit interview was conducted with Director, Darlene Skidmore. LPA provided a copy of the report, appeal rights, and a Notice of Site Visit (NOS). The licensee acknowledged that the NOS must remain posted in a prominent location for 30 consecutive days, visible to families and caregivers. |
