25 | On August 10, 2023 at 10:45 am, Licensing Program Analyst (LPA) Jessica Rubio arrived unannounced to the facility to conduct a case management visit due to an unusual incident report received for an incident that occurred on 6/30/2023, reporting child (C1) went to jump off of a play structure and was pushed by another child (C2). The fall caused C1 to injure their wrist and it was later determined C1's wrist was fractured.
LPA met with Director Jen Wilson and conducted a tour and census of the facility. During the visit, LPA interviewed Director, C1 and one staff (S1). Interviews revealed there were 5 children on the playground at the time of the incident with a teacher, (S1) providing supervision. The interview with C1 revealed C1 climbed to the top of the structure and went to jump of off when C2 pushed C1 causing C1 to land on their hands and arms. Interviews revealed staff applied first aid, contacted C1's parent and continued to observe C1 until C1 was picked up. Interviews revealed C1 continued to play and use their wrist until later in the day when C1 began complaining of pain and refused to use their arm. LPA observed the playground structure where the incident occurred. LPA observed the play structure to appear small, however, LPA also looked up the manufacturer's recommended age for the structure and it was between 24 months and 6 years old. LPA did advise Director that play equipment may need to be reassessed or replaced depending on the needs of the children and if Director finds that children are outgrowing the equipment. Director stated she understood. LPA determined there are no violations and no deficiencies were cited at this time.
An exit interview was conducted and a copy of this report and LIC 811 Confidential Names List was provided to Director Jen Wilson. Appeal rights were also provided and a notice of site visit was emailed to Director and must remain posted for 30 days.
