On the above date and time listed, Licensing Program Analyst (LPA) Keely Messerschmidt arrived at the facility for the purpose of delivering the complaint findings on the above-referenced allegations. LPA met with Director Weeratunga Costa. LPA toured the facility, conducted census, and verified facility staff and children enrollment. LPA discussed with Director the conclusion of the complaint investigation.

On April 27th, 2023, Community Care Licensing (CCL) received a complaint alleging that the Director is not on premises during hours of operation. In regard to the allegation, Director is never there when needed, LPA Messerschmidt conducted pertinent interviews with the Director, staff and other parties involved and was able to corroborate this allegation. Based on interviews, it was revealed that the Director is not on site daily. It was disclosed that the Operations Manager is on site three days a week and on these days the Director is not always available. It was also disclosed that a teacher assists with the front desk but does so from the classroom while in ratio, making it difficult to fully support center operations.

Based on the information obtained during this investigation, it has been determined that although the allegations may have happened or is valid, there is enough evidence to prove that the alleged violations did occur. Therefore, the allegations are SUBSTANTIATED.

An exit interview was conducted, and this report was reviewed with the Director, Weeratunga Costa, and a copy was provided. Appeal rights were discussed and provided during the exit interview.

A Notice of Site visit was given, and Director understands that it must remain posted for 30 days.

On the above date and time listed, Licensing Program Analyst (LPA) Keely Messerschmidt arrived at the facility for the purpose of delivering the complaint findings on the above-referenced allegations. LPA met with Director Weeratunga Costa. LPA toured the facility, conducted census, and verified facility staff and children enrollment. LPA discussed with Director the conclusion of the complaint investigation.
On April 27th, 2023, Community Care Licensing (CCL) received a complaint alleging that staff allowed infant access to an unsafe outdoor space. In regard to the allegation, an infant being seen on the big kids’ playground, LPA Messerschmidt conducted pertinent interviews with the Director, staff and other parties involved but was unable to corroborate allegation. Based on staff and child interviews, it was revealed that the infants are never on the big kids’ playground. Director and staff disclosed that once a child turns 2 years of age, this is when the child will transition to the preschool side, never before the age of 2 years. In addition, due to complainant being anonymous, LPA was unable to gather pertinent information in regards to the incident, which included the name of the child and the date it occurred.
