On November 13, 2024, at 8:15AM, Licensing Program Analyst (LPA), William Chancellor and Licensing Program Manager (LPM) Pauline Beschorner, arrived unannounced to Little Bears Tender Care (CCC) and met with Director (DIR) Carol Hadnot to discuss the investigative findings regarding the allegation’s listed above. On October 2, 2024, at 1:23PM LPA and LPM conducted a tour, took census of the CCC and gathered evidence relevant to the investigation. During the investigation, LPA made observations and conducted confidential interviews with three staff (S1-S3).

On September 19, 2024, a complaint was received alleging, children are left unattended without a staff member being present. Specifically, that twelve children were outside with no adult supervision when staff went inside to retrieve water for the children. Two of two interviews revealed that 2 staff were outside when one staff went inside to retrieve water bottles. Additionally, record review confirmed facility maintains, a one to twelve teacher to child ratios in each preschool classroom. LPA could not corroborate whether staff were present in the preschool classroom or on the playground as video surveillance was not available for review.

The second allegation stated that, facility staff failed to prevent a child from eating rocks. Specifically, that Child 1 (C1), was witnessed to swallow rocks and half a crayon. Three of three interviews did confirm that C1 tends to want to mouth or ingest multiple toys or foreign objects, but staff consistently redirect the child and have informed and provided resources to the parents for this behavior.

Based on conflicting interviews and record review, LPA is unable to corroborate the allegation children are left unattended without a staff member being present and facility failed to prevent a child from eating rocks. These allegation’s may have occurred; however, it is not supported, or proven by evidence. Therefore, the above allegations are unsubstantiated.

An exit interview was conducted, a copy of this report, and appeal rights, along with a Notice of Site Visit was provided to Director Carol Hadnot. The Notice of Site Visit must remain posted for 30 consecutive days in a prominent place, visible to families and caregivers.
