On December 19, 2024, at 12:01 PM Licensing Program Analyst’s (LPA’s), Courtnee Peebles and William Chancellor conducted an annual inspection as part of a compliance review. This is a combination childcare center and the other licensed programs are: School age and Preschool programs which were also inspected on this date. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:

· The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Menu
· The facility is operating with the limits as stated on the license.
· Ratios are being met during this inspection with seven infants and three staff
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards.
· There are no weapons present at the facility as stated by Carol Hadnot
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Drinking water is provided in the indoor activity space and in the outdoor activity space by public filtered water
· No Medications are stored currently at the facility
· Hazardous items are stored where inaccessible to children which include: Disinfectants, cleaning solutions

and other items that are dangerous
· All floors were observed to be clean and safe
· Highchairs and tables have broad-based legs and trays lock into place
· Changing tables are within arm’s reach of a sink; padding is at least 1” think with raised sides of at least 3” covered in washable vinyl or plastic
· Hand washing is completed before and after each diaper change and before feeding an infant
· Bathrooms were observed to be safe, sanitary and in operating condition; there is at least 1 potty chair for every 5 potty-training infants
· Playgrounds are enclosed by appropriate fencing and not free of hazards. During the facility tour LPA observed a toy kitchen with a broken attached conveyor belt, that is not age appropriate for infants in care. Toy kitchens for infants are allowable starting at age 18 months, however kitchens that possess a conveyor belt are allowable for children starting at 24 months of age. Furthermore, there was an additional kitchen for infants that was broken that possessed a sharp edge that could be harmful to infants in care. This poses a potential health and safety risk to children in care. A citation will be issued.
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin
· Food is not stored appropriately and protected from contamination, During the facility tour LPA observed a can of disinfectant Lysol on a push able cart on the top shelf next to three infant bottles that remained without lids. Director was reminded that toxic cleaning solutions shall not be next to feeding supplies, food, or other consumable items for children in care. This poses a potential health and safety risk to infants in care. A citation will be issued.
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record was reviewed and meets regulation requirements
· Disaster drills are conducted at least every six months – last drill conducted on 10/04/2024

A review of staff and children's records were conducted as part of this evaluation.
· Children’s records were found to be complete during this inspection.
· A staff member is present with current Pediatric CPR/First Aid which expires on 09/30/025
· Opening and closing staff member’s CPR/First Aid expires on 09/30/2025

· All staff present meet minimum qualifications for the position for which they were hired.
· A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that:


· Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall.
· The Licensee was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov

LPA discussed the safe sleep regulations with licensee/director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee/director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee/director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

An exit interview was conducted, and this report was reviewed with the licensee/director Carol Hadnot. Appeal rights were discussed and provided during the exit interview.

LPA and Director observed there aLysol disinfectant spral on the top shelf of a moveable cart, directly next to three infant bottles, that were without lids and had remaining milk, not preventing contamination. Director has been reminded to always complete a walk through of the playground and toys, before children have access to the space, to ensure they are in safe condition and age appropriate. Director will email LPA proof that both items have been removed from the infant playground.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
