On the above noted date and time, Licensing Program Analyst (LPA), Diana Brasel conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Applicant Guillermina Martinez Hernandez. Applicant is requesting to be licensed for 30 preschoolers in Classroom A and Classroom B. Hours of operation will be Monday - Friday 6:00 am - 8:00 pm, Saturday and Sunday 6:00 am - 6:00 pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant and director that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Guillermina Martinez Hernandez stated that they will be in a higher cabinet located in the kitchen for the assigned classroom of child in care. A fully equipped first aid kit is located in each classroom. There is an operational carbon monoxide detector on site located in each classroom. All required licensing documents were observed posted in each classroom and outside of Classroom B. Children will be signed in and out at their designated classroom.

LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 1070.64, which is sufficient to accommodate the requested capacity of 30 children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via bottled water, disposable cups and a drinking fountain. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance pending updated STD 850 sent.

LPA observed a total of 5 sinks and 4 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 30 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the office located upstairs.

Facility will provide breakfast, lunch, dinner, am snack and pm snack. There are two small kitchens located in classroom A and classroom B. Classroom A kitchen has a sink, refrigerator/freezer, toaster, and microwave. Classroom B kitchen has a sink, refrigerator/freezer, toaster, and convection oven. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via locked cabinet.

The facility currently has a fully fenced playground area. Fencing is chain link which is at least four feet high. The total square footage for all the outdoor activity space is 2784.81, which is sufficient to accommodate the requested capacity. Shade is provided via two small wind sails and a tree on playground A and one large wind sail on playground B. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a small structure with a slide located on playground A that is for children ages 2-8. There is adequate cushioning in fall zones of climber provided by rubber cushioning material on playground A and playground B has rubber cushioning material and artificial turf. Drinking water is available via pitchers and disposable cups. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Guillermina Martinez Hernandez was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

LPA reviewed with applicant the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.”

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Criminal Record Clearance - Child Care Centers:


Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The following corrections are needed prior to the issuance of the license:

1. Proof that the wall heater in Classroom A and Classroom B has been made inaccessible shall be
submitted.
2. Proof that the 5 small holes in the rubber cushioning material on Playground B have been repaired
shall be submitted.
3. A completed LIC 999 floor plan for the area above the classrooms shall be submitted.
4. LPA Brasel will obtain the updated STD 850 from the fire Marshall.

Applicant Guillermina Martinez Hernandez understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
