25 | On May 12, 2026, at 1:30 p.m., Licensing Program Analyst (LPA) Victoria Felix conducted an unannounced Case Management inspection regarding a self-reported incident. LPA met with Director Alma Hutcherson and disclosed the purpose of the inspection. LPA, accompanied by the Director, toured the indoor areas of the facility. There were six (6) children present, supervised by two (2) staff members.
Director reported that on 5/4/2026, Staff #1 (S1) and Staff #2 (S2) picked up twelve (12) children from a nearby School. Upon arriving at the facility 03:56 p.m. and entered at 4:00 p.m., an absence of supervision incident involving Child #1 (C1) was reported. Interviews were conducted with the Director, S2, and C1. LPA reviewed staff files and obtained copies of the facility roster, sign-in/sign-out sheets, and related documentation.
Director reported that on May 4, 2026, at 3:30 p.m., S1 and S2 went to pick up twelve (12) children from a nearby School. Upon arriving at the facility at approximately 4:00 p.m., staff escorted eleven (11) children upstairs. At approximately 4:05 p.m., while conducting a head count, Staff #2 (S2) realized that Child #1 (C1) was missing. S1 and S2 checked the restrooms and classrooms before discovering that C1 had been left inside the vehicle. At 4:09 p.m. C1 was back in classroom.
Based on interviews conducted, it was determined that C1 was left alone and without supervision inside a parked vehicle located in gated pin needed parking lot while asleep in a car seat for approximately nine (9) minutes. Director Alma Hutcherson stated that C1’s authorized representative was notified of the incident.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, a deficiency is being cited on the attached LIC 809D. This violation constitutes an immediate civil penalty for Absence of Supervision, Section 101229(a)(1). A $500 immediate civil penalty was assessed. Refer to LIC 421IM.
