32 | C1's cheeks and guide C1 away from the sink. Another staff member S2, after hearing a child cry, looked through a window and observed S3 use inappropriate motions on C1's cheeks multiple times. S3 was interviewed and acknowledged shoving C1's head away from the sink after giving multiple warnings. The incident was reported to the Director.
The Director reported the Unusual Incident Report to Licensing by phone and email on 6/18/2026. Director stated she discussed personal rights and appropriate staff conduct with S3 on the same date.
Based on S3's admission and interviews conducted during the investigation, it was determined that inappropriate discipline occurred, resulting in a personal rights violation.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, one (1) Type A deficiency was cited; see LIC90999D.
LPA Cindy Meier informed Director, Kristen Cobb, that this report dated 8/11/26 documents one (1) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Cindy Meier informed Director, Kristen Cobb to provide a copy of this licensing report dated 8/11/26 that documents the Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days.
