25 | On 03/21/2025 at 08:35AM, Licensing Program Analyst (LPA) Saul Zazueta conducted an unannounced case management inspection regarding a self-reported incident wherein a 3-year-old child was found unsupervised outside of a classroom, while the children and teachers were inside the facility. LPA met with Director, Aireen Turner. LPA discussed the purpose of the inspection and was led inside the facility.
During the inspection there were fifty-one (51) children in care, Director, and seven (7) staff.
Director stated, that on 03/07/2025, there were eleven (11) pre-school children with one (1) staff in the preschool classroom. Director stated that the incident occurred during the transition from the playground to the classroom. Director stated that staff claimed to have completed a “face-to-name log” prior to entering the classroom and counted every child. Director said that after a few minutes, knocking was heard on the door by two different staff members, who found a child to be left outside alone. Staff members delivered the child to the classroom and notified the Director of the incident. LPA requested to review the video footage from the day of the incident, but the footage was not accessible at the time of inspection. Interviews were conducted with Director, facility staff, and child that was left outside. Files and documents relevant to the complaint were reviewed and obtained.
Additional information is needed at this time. No deficiencies were cited during today’s inspection.
