On 10/30/2025 at 9:00AM Licensing Program Analyst (LPA), Saul Zazueta conducted an unannounced complaint inspection to deliver findings for the above allegation. Upon arrival, LPA met with Director, Dolores Elizabeth Guadarrama and discussed the purpose of the inspection and was led on a tour of the facility. There were forty-six (46) children, and seven (7) staff present during the inspection.

During the course of the investigation, LPA conducted interviews with the assistant director, staff and daycare parents. Photographs, surveillance footage of incident, incident reports and facility roster were obtained and reviewed by LPA.

Based on interviews conducted and records reviewed, it was determined that on 09/29/2025, Staff #1 (S1) did not provide sufficient supervision to prevent Child #2 (C2) from causing injuries to Child #1 (C1). LPA reviewed surveillance footage, which shows C2 using both hands to grab C1’s face on multiple occasions resulting in several scratches to C1’s cheeks.

S1 is observed, in the video, to be engaged in tidying up the classroom during the child interactions. S1 stated that they didn’t realize what had happened until they heard C1 crying and saw C2 with their hands on C1’s face. S1 stated they spoke to C2 about keeping their hands to themselves. LPA reviewed the surveillance footage with both the Regional Manager and the Assistant Director. After reviewing the footage, the Assistant Director stated that they see what’s wrong and that supervision could be improved.

Based on interviews conducted and records reviewed, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. Per California Code of Regulations, (Title 22, Division 12 & Chapter 1), one (1) Type A citation is being cited on the attached LIC 9099-D. A Civil Penalty in the amount of $250 for a repeat violation was assessed during today’s inspection.

LPA Saul Zazueta informed Director, Dolores Elizabeth Guadarrama that this report dated 10/30/2025 documents one (1) Type A citations which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Saul Zazueta informed Director, Dolores Elizabeth Guadarrama to provide a copy of this licensing report dated 10/30/2025 that documents Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted and this report was reviewed with Director, Dolores Elizabeth Guadarrama. A Notice of Site Visit was provided and must remain posted for 30 days.
