THIS IS AN AMENDED REPORT DELIVERED ON 7/17/23.
On 10/19/22 at 9:30 AM , LPA Annette Sutherland conducted an unannounced Case Management Inspection due to reported Lead Exceedance levels in the facility's water. LPA met with Director, Jennifer Sweet. There were 50 children and 10 staff members present in the preschool rooms. 
Water outlets reported with 5.5 ppb or greater lead exceedance levels as follows:

Sink A - Classroom  (8.120 UG/L)
Sink B - Classroom  (6.150 UG/L)
Drinking Faucet G Classroom (11300.000 UG/L)
Sink L- Classroom (7.130 UG/L)
Drinking Faucet V - Outside (787.000 UG/L)

LPAs observed  "Do not use" signs posted on the sink faucets during visit. LPA also observed removal of Drinking faucets G & V. Director stated Water testing inspection will be conducted again tomorrow and is not using the sinks until the results show the sink faucet is below the exceedance lead level.

LPA Annette Sutherland informed facility representative Jennifer Sweet that this report dated 10/19/22 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Annette Sutherland informed the facility representative Jennifer Sweet to provide a copy of this licensing report dated 10/19/22 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Director reported, sink faucets are in exceedance were replaced, and posted a "Do not use" sign until the results in the retest are lower than the exeedance level.

Based on water testing results and interviews, facility tested over the Lead Exceedance level at 5 sink faucet measuring one at 6.9ppb and the other at 11.0 ppb. Director stated the faucet has been used to fill filtered Brita water pitchers and provided the water to children in the past. This poses an immediate/potential health, safety or personal rights risk to children in care.

Deficiency cleared during visit. Water outlets have been made inaccessible drinking fountains (faucets) were capped off. Do not use signs posted on each outlet. Director states sink faucets were replaced and are pending retesting.

Based on water testing results and interviews, facility tested over the Lead Exceedance level at 3 sinks and 2 drinking fountains (faucets) measuring at 8.120ppb, 6.150 ppb, 11300.000ppb, 7.130ppb, 787,000ppb. This poses an immediate health, safety or personal rights risk to children in care.
