On November 2, 2023 @ 8:35AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced case management inspection in reference to a self-reported incident that occurred on October 26,2023 @ approximately 10:33 AM. A tour of the facility was conducted with Ivania Rodas, Site Director and Megan Stubbs, Ass't Director. Present today were a total of 35 children with 10 staff in 5 classrooms.
This incident involved a 2-year old child who wandered out of the facility unsupervised. This happened when Room #3 (with 12 children and 2 staff) were at the "Nature Space". Child was able to climb up a 16 ft. hill and walked approximately 120 ft. and was able to slip out of a fence that wasn't completely secured to the ground. The child was found by Megan Stubbs outside the church's office as she was on her way back to pick-up supplies. The church's office is located adjacent to the preschool building. The child was without visual supervision by staff for approximately 8 minutes.

Type A deficiency was cited today.

LPA Nancy Diaz informed the Site Director, Ivana Rodas that this report dated November 2, 2023 documents (1)Type A citation which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Nancy Diaz informed the Site Director to provide a copy of this licensing report dated November 2, 2023 that documents aType A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview was conducted with staff. A copy of this report, Appeal rights and Notice of site visit were given. Notice of site visit was observed posted and shall remain posted for 30 days.

Ms. Rodas stated that the Nature Space is currently closed until facility can install a fence around its perimeter. Ms. Rodas conducted individual conversations with staff and talked about Accountability and Supervision policy. She has also scheduled an all staff meeting to be held this week.

Based on documents, statements and photos submitted to the department, a 2-year old child wandered out of the facility without visual supervision of a staff.

Site Director will send a copy of the Agenda and sign-in sheet. She will also have staff sign a document indicating that Accountability and Supervision Policy was given.
