32 | Two of the six staff members reported that on the morning of June 17, 2026, a photo of C1 was uploaded to the KinderCare App, which showed no bruising, scratches, or scars on C1’s face or nose. Additionally, two of the six staff members stated that later in the afternoon (exact time unknown), they observed scratches on C1’s face and a scar beneath C1’s nose. Four of the six staff members acknowledged observing a scratch under C1’s nose; however, they could not explain how, when or where C1 sustained the injury.
It was further alleged that staff did not notify the child's authorized representative of an injury. All six staff members interviewed acknowledged that C1’s authorized representative was not informed of the bruises and scratches observed on the left side of C1’s face, nor of the scratches or scars located under C1’s nose.
Based on interviews conducted, and a review of photographs, the preponderance of evidence standard has been met. The allegations that the child sustained multiple bruises due to lack of supervision and staff failed to notify the child's authorized representative of an injury are found to be SUBSTANTIATED.
Per California Code of Regulations, Title 22, Division 12 & Chapter 1, one Type A deficiency and one Type B deficiency are being cited on the attached LIC 9099D.
LPA Vicky Williamson informed Facility Representative, Liliana Arredondo that this report dated September 1, 2026, documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Vicky Williamson informed the Facility Representative, Liliana Arredondo to provide a copy of this licensing report dated September 1, 2026, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
