32 | Patrick Ma reviewed the report with Katrina Hoffman, center director, and Lauren Greenfield, assistant director. The report was then acknowledged and signed by Ms. Hoffman.
Based on LPA interviews with staff and parents, and relevant documentation,the facility delayed the required distribution of these documents (copies of the LIC9099 licensing report and LIC9224) to parents. Staff interviewed during LPA Poindexter’s visit on 1/23/25 confirmed that completion of signed Acknowledgement of Receipt of Licensing Report (LIC 9224) forms was in progress but had been delayed at the decision of the center director. LPA observed several signed LIC9224 forms, as well as an 1/15/25 email to parents about the LIC9224 forms. Specifically, that “The individual copies for the parents, will be located in each Preschool classroom tomorrow evening. If you can sign the letter, we will be placing them in your files.” During interviews, Parents acknowledged seeing a document about the Type A citation at the “Parents Center” and some received the January email asking them to sign the LIC9224 form.
Based on the information obtained during interviews and documentation reviewed, it is determined that the allegation is valid because the preponderance of the evidence has been met, therefore, the allegation is found to be SUBSTANTIATED.

Exit interview conducted and report was reviewed with Lauren Greefield, assistant director. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were provided. |
