25 | On 3/05/2024, at 1:45 p.m., Licensing Program Analyst (LPA) Cindy Meier conducted an unannounced Case Management visit for the purpose of a follow-up on an Unusual Incident. LPA met with Director, Dinorah Juarez. LPA discussed the purpose of the visit and was led on a tour of the facility. There were eighty (80) children, and sixteen (16) staff present at the time of this inspection.
On 1/10/2024, the San Diego Regional Office received an Unusual Incident Report which stated on 1/5/2024 staff (S1) answered a personal phone call while supervising children and during the phone conversation forcefully tapped child (C1) on the shoulder and poked child (C2) on the chest with her index finger.
Director was informed of the incident on 1/8/2024 and viewed the classroom video footage of 1/5/2024.
During the visit, LPA conducted interview with Director, viewed seventeen (17) minutes of video footage of classroom 11 on 1/5/2024 and obtained relevant documents.
No citations are being cited during today’s visit.
An exit interview was conducted with Director, Dinorah Juarez. A copy of this report along with the Appeals Rights (LIC 9058) was provided. A Notice of Site Visit is provided and required to be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Director post Notice of Site Visit to the door at the entrance.
