32 | Records reviewed by the LPA, including S7’s personnel file, the facility’s LIC 500, and the facility’s internal system, identified S7 as a lead teacher. A review of S7’s transcripts confirmed that S7 has only three units, which does not meet the qualifications for a teacher. During a phone interview, S7 acknowledged being hired as a lead teacher.
Based on the information obtained through interviews and records reviewed, it was determined that S7, who does not meet teacher qualification requirements, supervised children in care. The preponderance of evidence standard has been met; therefore, the above allegation is found to be substantiated. California Code of Regulations, Title 22, Division 12, Chapter 1, is being cited on the attached LIC 9099D.
Exit interview conducted and report was reviewed with director Sheryl Crowel-Cook. A copy of this report, along with Appeal Rights (LIC9058), were provided. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
