It was alleged “Staff did not ensure that facility tables were clean”, LPA toured the facility 1/21/25, 2/12/25 and 3/17/25, LPA directly observed the tables being cleaned during lunch time. The tables were wiped down before the children sat down at the table and after the children finished eating. Interviews conducted provided that the facility is cleaned currently by staff and the director during business hours as well as after the facility has closed. Per interviews, staff clean tables prior to eating snacks and lunch. Tables are cleaned again once all children have finished eating. Per file review conducted, the facility does not have a previous similar allegation within the past year.

It was alleged “Staff did not ensure changing table was disinfected after each use”, interviews conducted provided staff have a process they follow when changing a child. The mat on the changing table is wiped down prior to placing the child on it. Once the child is on the mat, staff start the process of changing the diaper. The last step is to clean and sanitize the mat again then flip the mat over starting the same process for the next child to be changed. The written diaper changing policy was provided to LPA. Diaper changing training was provided on 1/22/25, and demonstrated by the Director to the staff.

Although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred, therefore the above allegations are found to be Unsubstantiated. Exit interview conducted and report was reviewed with the Director, Amanda Hernandez. A notice of site visit was given and must remain posted for 30 days.
