On October 20th, 2025 at 9:50 AM, Licensing Program Analyst (LPA) Oscar Picazo conducted an unannounced complaint inspection for the purpose of delivering findings regarding the above allegations. LPA met with Site Supervisor, Jonalynn Fuertes. At aproximately 10:55 AM, Area Supervisor, Patricia Jaimez arrived to the facility. There were 22 children in attendance being supervised by 9 staff members.

During the course of the investigation, interviews with the reporting party, Site Supervisor, nine (9) staff members, one (1) witness, and six (6) day care parents were conducted. A Facility roster, "Toddler Grams" (facility daily activity & diapering logs), a facility report and pertinent documents were obtained and reviewed.

It was alleged that staff did not ensure a child was changed in a timely manner resulting in a rash. The Site Supervisor and staff members stated they were not aware of or could not recall an incident where a child was not being changed in a timely manner that resulted in a rash.

During the interviews staff stated that all children who required diapering and or toileting services where either checked, changed and cleaned or taken for bathroom visits in accordance with the newly implemented diapering and toileting guidelines. These actions were recorded on the "Toddler Gram" facility diapering and daily activity logs of each individual child.

Based on interviews conducted and records reviewed, there were conflicting statements and no disclosures that corroborated the allegation that staff did not change a child in a timely manner that resulted in a rash and was found to be UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

It was also alleged that staff did not provide adequate supervision resulting in a child being hit by another child. The Site Supervisor and staff members stated that adequate supervision are always provided to the children both indoors and during outdoor activities. The facility incident report was reviewed detailing an incident where staff witnessed a day care child was playing with a toy vehicle and became agitated when another child unintentionally obstructed his path of travel. Due to the obstruction, the initial child began to scream and cry, dropped himself to the floor and bumped his head. The child was assessed and the authorized representative was notified.

Based on interviews conducted and records reviewed, there were conflicting statements and no disclosures that corroborated that staff did not provide adequate supervision resulting in child being hit by another child and therefore, the allegation is found to be UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

A Notice of Site Visit (LIC 9213) was given to Site Supervisor, Jonalynn Fuertes, and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal Rights (LIC 9058) were provided. An exit interview was conducted, and report was reviewed with Site Supervisor Jonalynn Fuertes.
