On 11/19/2024, at 3:30pm., Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced case management inspection for the purpose of issuing the correct Civil Penalty Assessment form. LPA met with Director, Ana King. LPA discussed the purpose of the inspection and was led on a tour of the facility. There were 60 children present with six (6) staff members.

On 10/29/2024, the facility was cited a Type A deficiency per California Code of Regulation, Title 22, Division 12, Chapter 1, section Criminal Record Clearance 101170(e)(1) due to Staff #1 (S1) did not have a criminal record clearance. Also, a $500 Civil Penalty was assessed and issued incorrectly on form LIC421IM Immediate $500 and Repeat Violations. The purpose of today’s inspection is to resubmit the $500 Civil Penalty on the correct form LIC421BG Background Check.

It is noted that the Director, Ana King, was provided form LIC421BG Background Check during the time of inspection.

Exit interview was conducted with Director, Ana King and Appeal Rights were provided. A Notice of Site Visit (LIC 9213) was provided and shall be posted for thirty (30) days from today’s date. LPA observed notice of site visit posted in the lobby.
