On 01/06/2026 at 10:00AM Licensing Program Analyst (LPA), Saul Zazueta conducted an unannounced complaint inspection regarding the above allegation(s). Upon arrival, LPA met with Director, Ana King and discussed the purpose of the inspection, the complaint process and was led on a tour of the facility.

During the course of the investigation, LPA conducted interviews with the director, staff, daycare children, and daycare parents. Photographs, timesheets, and facility roster were obtained and reviewed by LPA.

It was determined that on 10/17/2025, staff did not properly supervise daycare child #1 (C1) resulting in unexplained injuries. Based on staff interviews, the incident occurred on a play structure during outdoor play with two staff supervising a total of 22 children. Supervising staff reported to have been sitting at an activity table with children while the other was taking children to the restroom. Staff stated that they did not see the incident happen and knew about it only after the injured child notified them.

Although the director denied the allegation, stating that Teacher-Child ratios were maintained at the time of the incident, the staff were engaged in activities that shifted their focus away from the children on the play structure.

Based on interviews conducted and records reviewed, the preponderance of evidence standard has been met; therefore, the above allegation(s) are found to be SUBSTANTIATED. Per California Code of Regulations, (Title 22, Division 12 & Chapter 1) one (1) Type A citation are being on the attached LIC 9099-D.

LPA, Saul Zazueta, informed Licensee, Ana King, that this report, dated 01/06/2026, documents one (1)Type A citation(s), which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA, Saul Zazueta, informed the Licensee, Ana King, to provide a copy of this licensing report dated 01/06/2026 that documents Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted and this report was reviewed with Director, Ana King. A Notice of Site Visit was provided and must remain posted for 30 days.

Director stated that they have conducted supervision training on November 20th, 2025 that addresses different scenarios, including the scenario in which child #1 was injured, and how to prevent them. Director submitted a copy of the sign in sheet to the San Diego Regional Office on 01/06/2025.

Based on LPA interviews conducted and record reviews, the licensee did not comply with the above regulation, as the supervision provided by staff is ineffective in that a day care child caused injuries to another child in care, which poses an immediate health, safety and personal rights risk to children in care.
