On October 16, 2020, at 2:40 am, Licensing Program Analyst (LPA), Martha Malane conducted an unannounced video-conference to deliver findings regarding the above complaint allegation via FaceTime, due to the COVID-19 outbreak, and met with the director and assistant director. LPA Malane notified the purpose of the inspection and was led LPA on a tour of the facility. There were eleven (11) children in the Two’s Classroom with two (2) staff, seventeen (17) children in the Preschool Classroom with two (2) staff, twelve (12) children in the Pre-K classroom with one (1) staff and twleve (12) children witn staff one (1) staff in the TK classroom at the facility during the time of the inspection.

On July 24, 2020, CCL received a complaint alledging that the facility is co-mingling children in care. Interviews were conducted with the several day care parents, the director, assistant director and six (6) staff members. Staff time sheets, child sign-in/out sheets and daily classroom attendance records were reviewed.

Based on interviews and records review, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

LPA discussed and will provide the following documents to Director via email: LIC9099, LIC9099C, Technical Advisory and appeal rights (LIC 9058). The director was advised that acknowledgement of receipt of the report and appeal rights are to be received within twenty-four hours.

An exit interview was conducted.

It was alleged that the facility is operating out of ratio. During the course of the investigation, LPA conducted interviews with the director, the assistant director, six (6) staff members and several day care parents. Staff time sheets, child sign-in/out sheets and daily classroom attendance records were reviewed. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
